Accident Care Partners ("we," "us," "our"), a Revive Medical Care company, respects your privacy. This policy explains what we collect through this website and how we use it. It does not cover protected health information created during medical treatment, which is handled under separate healthcare-privacy notices.

Information we collect

When you submit a form, we collect the information you provide — such as your name, phone, email, date of accident, and message (patients), or your name, firm, email, phone, and client details (attorneys). We may also collect basic technical/usage data (e.g., IP address, browser, pages viewed) to operate and improve the site.

How we use your information

To respond to your inquiry, evaluate and coordinate care, contact you about your request, operate and secure the site, and comply with law. If you check the consent box, you agree we may contact you by phone, text, and email about your inquiry; message and data rates may apply, and you can opt out of texts anytime by replying STOP.

How we share information

We share information with service providers that help us operate (for example, our customer-relationship/CRM platform and our licensed clinical providers), with your attorney or legal team when you direct us to, and when required by law. We do not sell your personal information.

Data security

We use reasonable administrative and technical safeguards to protect your information. No method of transmission or storage is 100% secure, so we cannot guarantee absolute security.

Your choices

Your state privacy rights (CA · NV · AZ)

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. Depending on where you live, you may have additional rights:

California (CCPA / CPRA)

California residents have the right to know and access the personal information we collect and how we use it, to request deletion, to request correction, and to opt out of the "sale" or "sharing" of personal information. Because we do not sell or share your personal information, no opt-out is necessary — but you may still exercise your other rights by contacting us below, and we will not discriminate against you for doing so. Under California's "Shine the Light" law (Civil Code §1798.83), we do not disclose personal information to third parties for their own direct marketing. Medical information created during treatment is protected separately under HIPAA and California's Confidentiality of Medical Information Act (CMIA); this policy covers website and marketing data.

Nevada (NRS 603A)

Nevada residents have the right to opt out of the sale of certain personal information. We do not sell your information; to submit a verified opt-out request, contact us below.

Arizona

Arizona does not currently have a comprehensive consumer data-privacy statute. Arizona residents' information is handled under this policy, and we comply with Arizona's data-breach notification law (A.R.S. § 18-552).
